Home Health Care - Medical care at home
You want to remain independent at home and we will be there to help you! Whether you are recovering from illness or surgery or need help managing a chronic disease or condition, you can receive skilled nursing, physical therapy, occupational therapy, speech therapy, personal care and/or medical social services from our caring and encouraging staff in the comfort of your home. We will work with you and your doctor to develop a plan of care that is designed to meet your goals and help you feel more in control of your health. You and your family will have peace of mind knowing that skilled professionals are caring for you.
Palliative Care - Helps clients and families living with chronic or serious illness.
When you are facing a serious or chronic illness, you need relief from symptoms and help to tolerate medical treatments so you can carry on with everyday life. You also need to better understand your condition and choices for care to help you with decision-making.
Our palliative care team of specialized clinicians focuses on symptoms such as pain, shortness of breath, fatigue, constipation, nausea, loss of appetite, difficulty sleeping and depression through medication management, relaxation techniques, and coping strategies. Through psychosocial support and counseling we address emotional, social and spiritual issues to help you reach the best possible quality of life.
Home Health Telemonitoring - Daily monitoring of health status at home
Daily monitoring of your vital signs can identify changes in your condition that might become serious. A telehealth monitor in your home allows you to do this monitoring and report the results to a nurse who knows when those small changes need further attention. The small and easy to use monitor collects your vital signs and asks yes or no questions about your condition, then transmits the data via phone to our office where it is reviewed by our nurse. If any of the results indicate a problem, our nurse will call you, then take necessary action depending on the situation. We can also send reports to your doctor. Imagine the peace of mind for you and your family knowing that a health care professional is reviewing this information every day!
Hospice Care - For those living with a terminal illness and their families
All of us may someday be faced with our own or a loved one’s life limiting illness. If this happens to you or someone that you love, feel peace knowing that with the help of our caring hospice staff you don’t have to struggle alone to cope with the challenges and fears that occur. The last months, weeks, and days offer a time to laugh and love, to concentrate on the important issues and to say goodbye. The final moments should come with dignity and in peace. The hospice team includes professionals, para-professionals, and volunteers who are dedicated to caring for those living with a terminal illness and their families. They will assist by managing pain and symptoms, supporting your family, providing education on what to expect, and teaching care giving techniques. As a partner with you, your family and your doctors we will support you and your family every step of the way, not only by controlling your symptoms, but also by helping you to understand your treatment options and goals, all so you can have more control over your health and your life. If you or a loved one has been diagnosed with a serious or chronic illness, you may benefit from palliative care.
In-Home Support - Non-medical help to remain independent at home
Keep your independence at home with the help from our caring, dependable staff. Tailored to meet your specific needs, services may include help with dressing and bathing, transportation, meal preparation, housekeeping, laundry, grocery shopping and errands, lab services and more. You can schedule help for as little as a few hours a day or as much as 24-hour care. Your family will rest easy knowing that you have dependable help to keep you safe at home.
