Sacred Heart Hospital Diabetes Education Program

In an effort to bring health education and self-management to our patients Sacred Heart Hospital offers a diabetes education program that provides easy access to important information about living with diabetes.

Our Diabetes Program Coordinator has developed a diabetes program that is accredited by the American Association of Diabetes Educators (AADE). The program offers support and education on topics such as monitoring blood sugar levels, diet, activity and preventing complications and sick days.

About Diabetes
Diabetes is occurring at epidemic rates across all ages, socioeconomic statuses and races. More than 30 million people have diabetes. In Pennsylvania, almost 13% of the adult population has diabetes which has an annual cost of $245 Billion.  Every year an estimated 71,000 people in Pennsylvania are diagnosed with diabetes. 36% of Pennsylvanians have prediabetes; by attending a Diabetes Prevention Program, a prediabetic can prevent or delay the onset of diabetes.

Studies show that people who have attended Diabetes Self-Management Education (DSME) have better medical outcomes, such as lower A1C, weight loss, stable blood pressure and most importantly run a four times lower risk of developing the “complications” of diabetes – complications, such as heart attacks, blindness, kidney failure and amputations.  They also have an improved quality of life and less stress managing a chronic illness.

About Sacred Heart's Diabetes Education Program
Diabetes education is a lifelong process.  Since diabetes care is continually evolving, people with diabetes need ongoing education to stay informed about new tools and techniques to keep themselves healthy and productive.  Our program is continually being updated.  The Sacred Heart's Diabetes Education Program is designed for those who are newly diagnosed and for those who want to fine tune or learn new self-care strategies.  It is covered by most insurance companies; a small co-pay may be needed.

Personal one-on-one sessions are tailored to the individual needs of the patient.  The Diabetes Coordinator will adapt the educational information as needed for each patient.  A partial list of the educational material that is offered includes:

  • Monitoring blood sugar
  • Healthy eating
  • Being active
  • Taking medication
  • Problem solving
  • Reducing risks
  • Healthy coping

Our AADE accredited program emphasizes the need for patient management with generous support from the healthcare team. This team includes the patient and family or support people, the Primary Care Physician, Physician Resident, Registered Dietician, Registered Nurses, Social Workers and Case Managers.
