Transitional Care Facility
What is the Transitional Care Facility?
The Transitional Care Facility (TCF) is a 22 bed licensed unit, that provides 24-hour nursing services to residents after hospital discharge in a homelike environment for a period of time as determined by an interdisciplinary team. This includes physical therapy, occupational therapy, medicine, social services, nutritional services, nursing services and other departments as necessary. The population serviced by the department includes middle adult to geriatric care.
The Sacred Heart Hospital Transitional Care Facility is a full service department which provides comprehensive skilled nursing care and rehabilitative services to our resident population. The aim of TCF is to provide quality physical, psychosocial, emotional and educational support to residents and their families.
As a member of the Sacred Heart Transitional Care Facility, you are also a part of the Sacred Heart Health Care System. Sacred Heart Hospital is a 249 bed, independent Catholic Hospital, which provides services such as NICU, Open Heart Surgery, Neurosurgery, Orthopedics, Bariatrics, Pediatrics, Transitional Care, Medical/Surgical Care and Psychiatry.

- Physical Therapy

- Enterostomal Therapy
Skilled Nursing Services including:
- IV therapy services which includes antibiotics, blood administration, total parenteral nutrition and catheter care
- Pain management
- Comprehensive wound care
- Comfort care
- Respite care in coordination with Sacred Heart Hospice Program
- Assessment
- Resident/family education
- Tracheostomy care
- Management of tube feedings
- Bowel/bladder management
Other Services include:
- CHF teaching
- Post MI teaching
- Post CVA teaching
- Diabetic education
