School-Based Health Centers
Health Services for our Children
To help assure good health in young people, Sacred Heart Hospital, has developed several School-Based Health Centers. These centers can be found in Dodd, Jefferson, Mosser, Roosevelt, and Sheridan elementary schools and Harrison Morton and South Mountain middle schools. Children from birth to high school can conveniently use these centers as their "medical home".
Services
Our goal is to make children's health care accessible and affordable, and to provide health education to children and their families. Services provided at the health center include:
- Immunizations
- Early Periodic Screening Diagnosis and Treatment (EPSDT) physicals
- Lead and hemoglobin testing
- Daycare, school, camp and routine physicals
- Learner's Permits and Working Papers
- Sick care
- Chronic disease management
- Referral for special medical and dental care
- CHIP and Medicare application assistance
Hours
The health centers are open daily, Monday through Friday. Evening, weekend, and holiday coverage is provided by Sacred Heart Hospital and Health Call.
Staffing
Certified Nurse Practitioners (CRNP's) staff the health centers. Sacred Heart Hospital's Pediatric Services provides physician back-up of services.
Fee
The Sacred Heart School-Based Health Centers accept Aetna U.S. Healthcare, CHIP, ACCESS, Mercy, Three Rivers, Gateway, and other private health insurance plans.
Confidentiality
All services provided are confidential. Information will not be released without written consent of parent or guardian.
Enrollment forms may be picked up at the health center or the main office of the participating schools. For more information, please call 484-765-5976.
