Sacred Heart Hospice believes that everyone deserves to live in comfort with dignity throughout their lives.

When you or a loved one are facing a limited life expectancy due to serious illness, Sacred Heart Hospice offers expert compassionate and supportive care to assist you and your family in this difficult time. Hospice is a choice to have care aimed at providing comfort and support when curative measures are no longer effective.

    A specialized type of care for children and adults who have an advanced or life-limiting illness.
  • Focuses on meeting the needs and goals of the patient and family to help ensure that the end stages of a patient's life are meaningful and peaceful.
  • Offers a team of experienced professional staff and volunteers to give expert care, symptom control for comfort, emotional   and spiritual support to provide the best possible quality of life.

Who can receive hospice care?
Hospice care is available to anyone who has a life-limiting illness with a life expectancy of six months or less.  The patient's doctor and the hospice medical director agree with the plan to focus care on comfort and quality of life.  Sacred Heart Hospice cares for patients of all ages with any type of end stage illness such as cancer, AIDS, Parkinson's, stroke, Alzheimer's and advanced heart, lung, liver and kidney disease. 

When does hospice care start?
Sacred Heart Hospice begins providing care and support when a decision has been reached by the patient and family to pursue comfort care rather than seeking curative medical treatment.  

Who provides hospice care?
Hospice services are provided by a special team of experienced health care professionals and volunteers who see patients and families in their homes or nursing facilities.

Hospice services
Hospice care is given where the patient lives... In a patient's home, the home of a relative or friend, assisted living residences or a nursing home.  The same level of hospice care is provided anywhere the patient calls home.

Home Visits are scheduled intermittently, as needed for care, support and guidance. The hospice team is available to address needs 24 hours a day, seven days a week.

Hospice Inpatient Care for patients who are experiencing acute or severe pain symptoms. (Generally 3-5 days)

Respite Care for caregiver relief and rest (5 days) at a contracted nursing facility.

Medication and medical equipment related to the hospice diagnosis are also provided to the patient.

The patient does not need to be homebound to receive hospice services.

Medicare, Medicaid and most HMO's or private insurance plans provide a benefit for hospice care.  If there is no available benefit, we will arrange for care on an individual basis.

Sacred Heart Hospice serves residents in Berks, Lehigh & Northampton counties.
Hospice Care is available to all persons regardless of race, color, national origin, religious creed, age, sex, financial status or handicap.

A way to help Sacred Heart Hospice help others
Those wishing to assist our hospice program may do so through the volunteer program, fundraising    projects and activities or donations.  All Hospice volunteers complete a training program to prepare them for patient and family services.  Volunteers may also serve as clerical support staff, fundraisers or public speakers.

We also could not continue to serve our patients and families who cannot afford to pay for care without the help of gracious donors who see the need and generously respond. 

In the name of those in need, we gratefully accept contributions which may be made in recognition of a current patient or as a memorial to a deceased individual.  All contributions are tax deductible and acknowledged by the Sacred Heart Foundation.
