Sacred Heart Visiting Nurses is a non-profit, hospital-based home care agency which serves patients of all ages in Berks, Lehigh, and Northampton counties. Our full-service capabilities enable us to meet the patient’s total home care needs. Sacred Heart Visiting Nurses provides services such as skilled nursing, physical therapy, occupational therapy, speech therapy, social services and home health aide services. Specialty programs include maternal and child health, pediatrics, wound care, enterostomal therapy, cardiac care, diabetic care, and oncology care. Our multi-disciplinary staff implements an individualized plan of care designed to achieve positive clinical outcomes. We are committed to the highest standards of excellence. Sacred Heart Hospice
Sacred Heart Hospice is a certified provider of the Medicare Hospice benefit and is approved by numerous insurance plans. Our goal is to enable terminally ill patients, young and old, to live life to the fullest in their homes among family and friends. Our multi-disciplinary team includes physicians, nurses, therapists, medical social workers, clergy, other counselors and volunteers. This team develops an individualized plan of care that focuses on physical, emotional and spiritual comfort during the final stages of illness and bereavement. Ongoing communication occurs among the hospice team to anticipate and meet the special needs of the patient and family.
Sacred Heart Palliative Care
The Palliative Care Program at Sacred Heart Home Care focuses on reducing the severity of the symptoms of a disease and alleviating the side effects of curative treatments. Our team of palliative care providers will work to manage pain, enhance the quality of daily life, and to ensure that you and your loved ones understand your condition and your medical options.
Sacred Heart Personal Family Care
Personal Family Care is designed to enhance the quality of life for our patients, their families and caregivers. Personal care staff is available to provide support for families and caregivers on a fee for service basis. Our home health aides are caring, trained individuals and receive supervision from our nurses. Home health aides assist with personal hygiene (bathing, skin care, dressing and grooming), supervise activities, take and record vital signs, prepare meals and help with feeding. They can also perform light housekeeping, shopping, errands, and offer companionship. We provide families and caregivers respite in their home, hospital, assisted living or skilled nursing facility.
Sacred Heart Pharmacy
Sacred Heart has a full-service pharmacy, which provides high-quality, cost-effective services including outpatient prescription dispensing, patient consultations and infusion therapy services. For outpatient prescription dispensing to assisted living facilities and children’s camps, Sacred Heart Pharmacy utilizes the Medication on Time® packaging system, a 30-day prepackaged supply of medications for clients which ensures accurate administration of medication and patient compliance. Sacred Heart Pharmacy’s infusion therapy services include: Total Parenteral Nutrition (TPN), antibiotics, pain management, and hydration therapy. Sacred Heart Pharmacy is open daily from 8:00 a.m. - 6:00 p.m., Monday - Friday, and Saturdays from 9:00 a.m. - 12:00 p.m. Sacred Heart Pharmacy has a pharmacist on call 24 hours a day, 7 days a week, to support all aspects of our services.
Sacred Heart Medical Equipment & Respiratory Services
Home medical equipment is prescribed for patients with chronic diseases or disabilities that limit their activities and for newly discharged patients who are continuing their prescribed rehabilitation therapies at home. Hospital beds, wheelchairs, and ambulatory aids are available in a variety of styles. Respiratory home care allows both chronic and stable cardiopulmonary patients to return to a more normal lifestyle while continuing their prescribed therapy. Sacred Heart Medical Equipment specializes in oxygen, aerosol and humidification devices, and CPAP or BiPAP® units.
