In March 2002, the Joint Commission launched a new patient safety campaign called Speak Up. Since then, the Joint Commission has launched Speak Up programs on wrong site surgery; organ donation; infection control; and medication mistakes. Medical Errors
Medical errors happen when something planned as part of medical care doesn’t produce the desired results, or the incorrect plan of action was instituted initially. Medical errors can occur anywhere in the healthcare system: hospitals, clinics, outpatient surgery centers, doctors’ offices, nursing homes, pharmacies, and patients’ homes. Errors can involve medicines, surgery, diagnoses, and equipment lab reports, and they can happen during even the most routine tasks. Fall Prevention
Each year thousands of men and women are disabled, sometimes permanently, due to falls. With age, many people become more susceptible to falls because of health conditions that cause them to lose their sense of balance. However, there are steps that can be taken by you and your healthcare facility to reduce falls as much as possible.
