ALLENTOWN, PA – Sacred Heart Hospital is cordially inviting the community to attend the Grand Opening Celebration and RibbonCutting Ceremony for Sacred Heart Hospital’s new Transitional Care Facility. The celebration and ceremony will be held on Wednesday, June 24, 2009 from 3:00 p.m. – 5:00 p.m. Attendees can meet our expert transitional care staff, learn about our enhanced facility and enjoy light refreshments.

Sacred Heart Hospital’s Transitional Care Facility (TCF) is a licensed 22-bed facility that provides individualized comprehensive skilled nursing care and rehabilitative services in a home-like environment. Our residents range in age from adult to older adult. In TCF, we offer a wide range of services such as nursing, nutrition, and social services. Residents may also receive a variety of therapies such as physical, occupational, speech, respiratory, enterostomal, and recreational therapy. Our interdisciplinary team approach ensures that we provide the highest quality of care. Residents and their families also receive psychosocial, emotional, and educational support, and families are encouraged to participate in a resident’s treatment plan.
In December 2008, TCF was rated the highest rating of Five Stars by the U.S. Centers for Medicare and Medicaid Services, under the U.S. Department of Health and Human Services. Sacred Heart’s TCF was one of only two facilities in Lehigh and Northampton Counties to receive this honor. The Five-Star Quality Rating System was created to help consumers, their families, and caregivers compare nursing homes more easily and help identify areas about which you may want to ask questions. This is the first-ever federal public ranking of 15,800 Medicare and Medicaid-certified facilities. The government used data from health inspections, staffing, and quality of care measures to arrive at the ratings released on December 18, 2008. The health inspections assess a nursing home’s health care services in compliance with federal and state rules.
