Sacred Heart Hospice

Sacred Heart Hospice is a component of Sacred Heart Home Care Services, which also includes Sacred Heart Visiting Nurses, Sacred Heart Pharmacy & Infusion Services, and Sacred Heart Medical Equipment & Respiratory Services.

Sacred Heart Hospice is a certified provider of the Medicare Hospice benefit and is approved by numerous insurance plans. Our goal is to enable terminally ill patients, young and old, to live life to the fullest in their homes among family and friends. Our multidisciplinary team includes physicians, nurses, therapists, medical social workers, clergy, other counselors and volunteers. This team develops an individualized plan of care that focuses on physical, emotional and spiritual comfort during the final stages of illness and bereavement. Ongoing communication occurs among the hospice team to anticipate and meet the special needs of the patient and family. Sacred Heart Hospice is licensed by the Pennsylvania Department of Health and accredited by The Joint Commission.
